There is a simple action plan, the 3rd player must make the tackle "in front" - not behind....this is the problem as the 2 tacklers involved already are in front or side, so the bloke at the back and low is "always" going to bring an awkward position for the ball carrier.
If coming from in front can still help contain the attacker and its main priority, to slow the play the ball down and control ruck speed, without the danger of snapping backs, knees and ankles.
